Encompass is one of three Population Health pilot projects in Minnesota, funded by a grant from the LeadingAge Minnesota Foundation. Our goal is to help across every domain of wellness, no matter what your current stage of aging is and to minimize barriers that may prevent aging adults from maximizing their quality of life.

Centra Care

Part of the CentraCare health system, the team offers a full range of health services, as detailed on our website.

Olivia Hospital & Clinic

Part of the Health Partners health system, the clinic offers a full range of health services, as detailed on their website.

Western Mental Health Center

Provides behavioral health and substance abuse services and is co-located in the CentraCare Redwood location.

Renville County Public Health

Provides a Medicare-certified and licensed home care agency as well as connections to resources and services.

Lower Sioux Health Care Center (LSHCC)

We provide healthcare services to members of the Lower Sioux Indian Community and employees of Lower Sioux businesses.

MN River Area Agency on Aging

Provides advocacy, information, resources, and assistance so that older adults can maintain the lifestyle of their choice.

Southwest Health and Human Services

Provides a variety of public health and social services to elevate and evaluate community health and wellness.

United Community Action Partnership

Helps connect people and the resources they need to change lives and eliminate poverty.

Nutrition

We work with on-site registered dieticians at CentraCare Redwood and Olivia Hospital & Clinic to offer pre-diabetes classes, nutrition classes, and more, as part of a comprehensive nutrition education resource for the collaborative.  

In addition, we can connect you with United Community Action Plan who assists with Nutrition Assistance for Seniors (NAPS), mobile delivery in Renville County for Food Shelf services as well as helping with SNAP applications for food assistance to eligible individuals.

Connection and Referral to appropriate care options beyond these alternatives, including home care, assisted living, memory care, care suites, skilled care, and hospice: 

CentraCare Redwood Home Care has an established team of RNs, LPNs, and home health aides to provide extra care during a temporary timeframe or on an ongoing basis tailored to patients’ unique needs.  
Renville County Public Health provides a Medicare-certified and MDH-licensed home care agency.  
We will approach skilled nursing facilities: Renville Health Services in Renville and River Valley Health and Rehabilitation Center in Redwood Falls to be part of the collaboration. 
Vista Prairie at Garnette Gardens offers independence plus, assisted living, memory care, and care suites options. 
CentraCare Redwood provides hospice care services as well as Olivia Hospital and Clinic Hospice that have an established team of Registered Nurses, home health aides, and a social worker.
